DOT & CDL Physicals and Medical Cards

What is involved with DOT Physicals and CDL Physicals?

• Vision: Can be completed with or without glasses or contacts.
  • See at least 20/40 with left eye
  • See at least 20/40 with right eye
  • See at least 20/40 with both eyes
• Blood Pressure and Pulse: The federal motor carrier and safety administration (FMCSA) requires a blood pressure under 140/90.
• Urinalysis: tests for blood, sugar, and protein which may be for underlying conditions such as diabetes
• Hearing: Must be able to hear a forced whisper from 5 feet away
• Heart and lungs check for normal sounds and function
• Good grip strength and normal neurologic function and reflexes

What you may need to bring with you?

• Medication List
• Corrective Lenses & hearing aids if needed.
• Valid US State Drivers License: note cannot use license from US territories like Puerto Rico
• Clearance Letters & Lab results for specific conditions
  
  • Recent A1C results for diabetics
  • Insulin Clearance Form MCSA-5870: filled out before your visit by your treating doctor
  • Vision Evaluation Report MCSA-5871: filled out before your visit by ophthalmologist or optometrist before your visit if you cannot meet vision standard above.
  • Sleep Apnea annual compliance reports
  • Cardiologist annual clearance letter for heart conditions and surgeries
  • Pulmonologist annual clearance letter for chronic lung diseases
  • Mental Health annual clearance letter if on psychiatric meds (depression, ADHD, etc)
  • Pain Management annual clearance letter if on any narcotics
  • Neurologist annual clearance letter for peripheral neuropathy or history of strokes or seizures in past 5 years
