Florida CDL Medical Card Reporting Steps

1. Enter Florida DMV Website (https://services.flhsmv.gov/CDLMedCert/) & select "I Agree" option.
2. Enter Florida Driver License Information, last 4 digits of social security number, and date of birth
3. Click "Self-Certify"
4. Select "NO" if you do not need waiver or Skills Performance Evaluation
5. Select 1 of the following options
   1. Select "A" if interstate driver (most cases)
   2. Select "C" if certifying for only intrastate.
6. Enter phone number & email 
7. Copy information from Medical Examiner's Certificate into matching fields on page
8. Take a picture of your medical card.
9. Click submit and you are done.
